Social Work Manager

Location: San Jose, California

Duration: Full-Time

We are seeking an experienced and compassionate Social Work Manager to join our team in San Jose, CA. This role offers the opportunity to lead and mentor a dedicated group of social work professionals while ensuring exceptional care for frail and elderly participants in our program.

Key Responsibilities

• Lead and manage Social Work Supervisors, Social Workers, Behavioral Health Specialists, and Social Work Assistants, including hiring, training, supervising, and coaching.
• Oversee the quality and effectiveness of social work assessments and active participation in interdisciplinary team (IDT) meetings.
• Ensure high-quality delivery of social work interventions as outlined in participant care plans and respond to urgent needs.
• Maintain compliance in all social work areas including documentation, care plan development, participant rights, and grievance processes.
• Collaborate with Behavioral Health Specialists to design and oversee programs such as support groups, counseling, and family education.
• Support end-of-life care coordination and provide compassionate guidance to families.
• Conduct home visits when necessary.

Qualifications

• Master’s degree in social work (MSW) – required
• Social Work Licensure (LCSW or equivalent) – preferred
• Minimum 2 years of supervisory or leadership experience in social work – strongly preferred
• At least 1 year of experience working with frail or elderly populations
• Strong clinical, organizational, and communication skills across diverse populations
• Valid state driver’s license with a clean DMV record
• Proof of recent vehicle insurance
