About the Role
The Social Work Manager leads and supports the Social Work team, ensuring that services are delivered with compassion, consistency, and respect. This role is part of the leadership team and works closely with colleagues and community partners to strengthen programs and uphold our mission.
What You’ll Do
- Guide and support the Social Work team, including supervisors, social workers, behavioral health specialists, and assistants
- Oversee social work assessments and team participation in care planning
- Ensure that participants receive the support outlined in their care plans, responding to urgent needs as they arise
- Maintain compliance with documentation, assessments, and participant rights processes
- Work with behavioral health staff to offer counseling, support groups, and family education in partnership with community professionals
- Provide support during end-of-life care planning and coordination
- Conduct home visits when needed
What We’re Looking For
- Master’s degree in Social Work (MSW) required
- Licensed social worker preferred
- At least 2 years of supervisory or leadership experience in social work strongly preferred
- Minimum 1 year of experience supporting frail or elderly individuals
- Strong clinical, communication, and organizational skills
- Valid driver’s license and proof of insurance
Nice to Have
- Experience with interdisciplinary care teams
- Familiarity with community-based programs and services
