The Resident Care Manager (AL/MS) is responsible for the monitoring of the health and wellness of our residents. The responsibilities include ensuring the overall well-being of the residents, assisting in training and monitoring LVNs, CNAs and CMAs and maintaining professional and timely communication with all resident support services and families.

The Resident Care Manager (AL/MS) is responsible for the monitoring of the health and wellness of our residents. The responsibilities include ensuring the overall well-being of the residents, assisting in training and monitoring LVNs, CNAs and CMAs and maintaining professional and timely communication with all resident support services and families.
- Provides services in accordance with state assisted living standards and nurse practice act and maintains compliance to all personnel policies, established community policies and procedures, and Federal/State regulations and standards.
- Monitoring each resident’s medication/treatment profile to ensure that each medication/treatment is administered as ordered, is available and is documented correctly.
- Reconciling medication deliveries for accuracy and timeliness.
- Observing/evaluating, monitoring, and following up with residents who have experienced any fall, accident, or incident.
- Collaborating with AL/MS Director in analyzing care/clinical outcomes in assisted living and/or memory care.
- Assisting in the coordination of ancillary supportive services such as Home Health, Hospice, Outpatient Rehab, Mental Health, provider visits, etc.
- Responsible for staffing and scheduling of LVNs, CNAs and CMAs including finding coverage for call-ins and other shifts, as necessary.
- Developing relationships with the residents and their families using a consistent caregiver approach; supporting the resident’s physical, spiritual, emotional, and clinical needs and focusing on placing the resident first.
- Participates in pre-admission and move-in assessments, reviews, and revise assessments with any change in resident’s condition and at frequency determined by the state.
- Completing observations and evaluations of residents for change in clinical condition.
- Develop and revise service plans for residents based on needs assessments.
- Identify the on-going needs of residents in order to maintain the highest achievable level of well-being, ensuring all medical needs are being met.
- Supervises CNAs, CMAs and LVNs.
- Assist in hiring, orientation, training and disciplinary process of CNAs, CMAs and LVNs.
- Licensed Vocational Nurse and ability to practice nursing in the state of Texas.
- Current CPR.
- A minimum of one year’s experience in assisted living, home health or long-term care preferred.
- Experience with electronic medical records/PCC preferred.
- Exhibits a passion to care for seniors and is a strong resident advocate.
- Ability to work hands on when unable to find coverage.
- Schedule may be altered depending on staffing needs.
- Management experience required; Assisted Living and Memory Support preferred.
