This is an adult renal transplant social work position. The role includes a range of clinical social work and case management services in the inpatient and outpatient setting. This social worker will care for patients in the transplant program during all phases of transplant including transplant evaluation, transplant admission, and transplant follow-up. The social worker will address with the patient/family any barriers to transplant and then continue to work with them on transplant readiness, compliance concerns and any post-transplant needs. In this role the appropriate candidate will serve as the lead discharge planner from the inpatient arena.

Responsibilities
- Psychosocial assessment as part of transplant candidacy for the program.
- Counseling and crisis intervention.
- Facilitation of the use of hospital and community services.
- Resource information and referral, and program development.
- Psychosocial consultation to multidisciplinary team.
- Facilitation of kidney transplant support group.
- Master’s in Social Work from a school of Social Work from a program registered by the NYS Dept. of Education or determined to be substantially equivalent by the commissioner's regulation as required.
- A valid NYS LMSW or LCSW License and Registration is required. If employee has a limited permit, they must have an executed Memorandum of understanding with a licensed Social Worker for the purposes of supervision and must obtain LMSW within 6 months of employment.
- Previous experience with transplant patients and patients with life threatening illness preferred.
