What You Will Do:

  • Assess patient/family social and emotional factors to estimate their potential to cope with terminal illness and death.
  • Assist the physician and other hospice team members to understand the emotional and social factors related to the patient’s health problems so that each patient’s plan of care fosters wellbeing and human dignity.
  • Assures that social service/counseling activities are integrated and coordinated within hospice.
  • Identify and address comfort care needs in collaboration with other health care team members.
  • Assist patient and family in utilizing community resources.
  • Assess patient family financial and insurance status and assist with appropriate resources as indicated.
  • Participate in Interdisciplinary Team (IDT) meetings to establish, review, and revise plan of care for each patient.
  • Support, instruct, and educate patients, families, and others involved in patient’s plan of care.
  • Regularly re-evaluates the patients and family’s psychosocial needs while consulting and collaborating with IDT.
  • Demonstrates knowledge of clinical social work.
  • Treats patients and their families with respect and dignity. Identifies and addresses psychosocial needs of patients and family.
  • Coordinates and directs psycho-social plan of care to ensure patients' needs are met and hospice policy is followed.

About You:

  • Must have a Master of Social Work (MSW) degree from a school of social work in psychology, sociology, or other related filed to social work
  • At least one year of experience in similar setting required
  • Excellent observation, verbal and written communication skills, problem solving skills
  • Current driver’s license and car insurance
  • Travel is required throughout the majority of the day. Candidate be willing to operate personal car necessitated by nature of job
  • Familiar with community resources as they relate to hospice

Physical Demands:
The position requires sitting, standing, walking, bending, kneeling, and stooping, crouching, reaching,
and climbing, use of telephone as well as computer throughout most of the day. The employee must have
adequate (with correction) close vision, distance vision, depth perception, and the ability to focus. The
employee must occasionally lift or move objects and patients weighing over 25 lbs. or more.
