Description
Pediatric Social Worker needed for full-time, temporary position in Camden County.
Responsible and accountable for the provision of social work services to residents, their families, and significant others to assist them in coping with the impact of illness/disability and extended care placement, and to achieve maximum benefit from health care and community services. The Social Worker shall ensure that each resident is provided with coordinated care and services to attain or maintain the highest level of physical, cognitive, emotional, spiritual, and psychosocial well-being, following an individualized comprehensive assessment and an interdisciplinary plan of care.
Position Requirements
NJ LSW and a minimum of two years pediatric health care experience, preferably with special health care needs. Computer literacy. Assessment skills. Collaborative Care Planning and Coordination. Advocacy. Community Resource Referral. Supportive Counseling/Education. Discharge Planning.
