SUMMARY:

The Assistant Clinic Manager and Care Coordinator is responsible for supporting the Clinic Managers with daily clinic operations and the care coordination of patients with chronic illnesses or who are high-utilizers of health care. This position is an integral part of the Patient-Centered Medical Home care team and works collaboratively with others to support optimal patient outcomes.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

The following list reflects the general duties considered necessary to fulfill the principal functions of the job and shall not be considered a detailed description of all the duties required that may be inherent in the position.

  • Supports the Clinic Manager in supervision of all support staff and volunteers working at the clinic:
    • Supports in the overseeing of staff.
    • Responsible for coordination of clinic flow.
    • Works with Clinic Manager to assure that staff and volunteer personnel files are complete and have up to date information according to licensing and other regulations.
    • Supports in processing employee timesheets and submitting them to accounting for payroll purposes in compliance with SBNC policy and procedures.
  • Supports the administrative functions of the clinic so that it runs smoothly and efficiently:
    • Confirms that all clinic equipment is properly maintained and cleaned.
    • Works with Clinic Manager to assure that clinic supplies are adequate.
    • Assures that all appropriate report corrections (i.e. NextGen data, CAIR Immunization) are submitted on a timely basis.
    • Supports QI and Compliance Manager with implementation of procedures as agreed to by the MQI Committee.
    • Arranges for routine maintenance of clinic site.
  • Provides Care Coordination Services to defined populations:
    • Analyzes population lists in i2i for patients who qualify for care coordination and enrolls qualified patients into care coordination program
    • Provides care coordination appointment and works patient to complete My Care Plan and develop patient-centered health improvement goals. Attends primary care provider appointment and captures any additional information from the primary care provider into My Care Plan.
    • Follows up with patients via text, phone, e-mail or via patient portal to support patient in achievement of their goals.
    • Works with care team to ensure patient is receiving needed referrals, support and other services to support patient’s goals.
  • Maintains quality customer service.
  • May need to cover at other sites if needed.
  • Other related duties as assigned.

  • Demonstrates understanding of and observes all SBNC policies, procedures, rules and regulations.
  • Demonstrates successful work-related behaviors.
  • Attends all required SBNC meetings and trainings.
  • Interacts with patients, physicians, staff, vendors, and visitors in a positive manner that reflects SBNC’s mission and philosophy.

QUALIFICATIONS AND EDUCATION REQUIREMENTS:

Education: High School diploma or GED equivalent.

Experience: One year of clinical setting experience required. One year experience in a non-profit clinic/community service setting or related experience and/or training; or equivalent combination of education and experience. Coordination of clinic flow experience preferred.

Knowledge & Skills: Knowledge of non-profit clinic flow coordination preferred. Knowledge of medical terminology, Medi-Cal and other payer sources preferred. Must be able to learn quickly, in a fast paced environment. Must possess interpersonal skills to interact diplomatically with staff and difficult clients. Must be able to work with a very diverse group of patients in a non-judgmental manner. Computers skills and working knowledge in the use of standard software such as Windows, Word, Excel and PowerPoint.

Must possess strong verbal and organizational skills. Bilingual in English and Spanish preferred.

Physical Demands: May be required to lift objects weighing up to 25 pounds. Sitting in a normal seated position for extended periods of time. Reaching by extending hand(s) or arm(s) in any direction. Finger dexterity required to manipulate objects with fingers rather than with whole hand(s) or arm(s), for example, using a keyboard. Communication skills using the spoken word. Ability to see within normal parameters. Ability to hear within normal range. Ability to move about.

Salary Range: $74,000 - $78,000 annually
