Senior LIFE is currently seeking a Home Care Coordinator to fill a full time, permanent, position at Senior LIFE Indiana. The ideal candidate will hold a valid driver's license, and must have two year's case management experience working specifically with an elderly or aging population.

Responsibilities of the Home Care Coordinator
- Assesses and coordinates the homecare needs of a frail elderly population and identifies and develops specific plans of care in combination with the IDT.
- Visiting members in their homes and supervising approved care/services
- Conducting ongoing periodic assessments, ensuring the current care plan is still the best course of action
- Attending family and Senior LIFE team meetings to update participants on members’ status and needs
- Documentation/Data Collection
- Perform supervisory visits as needed for support staff to ensure quality service is being provided.

Requirements of the Home Care Coordinator
- Must have a minimum of 2 years case management experience
- Must have a valid Driver's License
- Degree in health related field preferred
- Experience working with the elderly population
- CNA license required
