Columbiana County Board of Developmental Disabilities
Position Title: Early Intervention Service Coordinator
Normal Working Hours: 40 hour week, flexible and variable to meet the program needs
Normally M-F, 8:00AM-4:00PM, late visits may be necessary on occasion
ESSENTIAL FUNCTIONS
Establish and maintain positive relationships with assigned families through regular home visits and monitoring, serving as single point of contact to assist families throughout their Early Intervention (EI) journey beginning with initial contact through transition at age 3
Actively participate as a member of Columbiana County’s Evidenced–Based Team
Share and ensure parents’ rights within EI system and complete required consents
Observe developmental skills of infants and toddlers; conduct screenings as appropriate
Gather medical, developmental and social history information to present to team
Coordinate eligibility and enrollment process including coordination of developmental evaluation and assessment
Conduct family-directed assessment to identify family’s priorities
Provide information about community resources and make relevant referrals
Coordinate and facilitate the development and review of the Individualized Family Service Plan (IFSP) that identifies needed EI services and their funding sources
Monitor the Timely Receipt of Service (TRS) and ongoing provision of services as indicated in the IFSP
Coordinate transition into early childhood services at age 3 and facilitate a Transition Planning Conference (TPC) with the Local Education Agency (LEA)
Maintain accurate documentation of all contacts with families and providers and complete data entry of all EI activities into the Early Intervention Data System (EIDS) and Brittco.
Collaborate with the medical community, educational organizations, and public and private agencies
OTHER FUNCTIONS
Attend meetings and training sessions, including state and local meetings as assigned
Adhere to the ORC, OAC, and to CCBDD policies and procedures
Perform such other duties and responsibilities as assigned by administration
WORK ENVIRONMENT
Requires travel throughout the county to conduct home visits or attend meetings
May be exposed to infectious disease
May work with individuals with behavioral disabilities who display challenging behaviors
May be exposed to chemicals commonly found in an office environment (i.e., toner, correction fluid, etc.)
REQUIREMENTS
Associate’s Degree in Early Childhood Education or related field
Eligible for Service Coordinator credential issued by the Ohio Department of Developmental Disabilities (DODD)
Possess knowledge of available community resources
Ability to interact in a sensitive and supportive manner with families and guardians
Ability to efficiently plan to meet tight deadlines.
Ability to negotiate and manage conflict
Ability to use coaching strategies to support parents
Ability to work flexible hours on occasion, including evenings
Ability to attend required in-service trainings as offered by DODD
Evidence of a valid driver’s license in state of residence, good driving record and full liability coverage on any vehicle used for transportation
Excellent interviewing and documentation skills
Good organizational and time-management skills
Exceptional communication skills (verbal, written, and interpersonal) for effective interaction with internal and external customers
Self-starter with ability to work independently
Possess computer skills in various word processing and database management programs
Bureau of Criminal Identification and Investigation (BCII), Federal Bureau of Investigation (FBI), and abuser registry clearances
Maintain all training requirements as per Board Policy

Pay: $36,000.00 - $45,000.00 per year
