Niagara Falls Memorial Medical Center is looking for a Licensed Clinical Social Worker/Mental Health Counselor/Creative Art Therapist to work as part of our outpatient clinic team.

This individual will be responsible for assessment of patients’ mental health functioning and the development and implementation of a plan of care to meet clients identified needs. Working on a multidisciplinary team with responsibility for performing individual, group, family and crisis interventions to stabilize symptoms and maximize functioning. Identification and clinical management of high risk – high need patients. Collaboration with community based service providers in an effort to assure comprehensive treatment. Responsible for mental health clinical care and direct patient coordination of services.
