We're currently seeking a full-time Social Worker to participate in the development and implementation of a safe, timely discharge plan and intervenes with any barriers which may affect the discharge planning process. Social Worker supports and educates the interdisciplinary team regarding appropriate community resources and facilitates linkages for continuity of care.

Overview
We're currently seeking a full-time Social Worker to participate in the development and implementation of a safe, timely discharge plan and intervenes with any barriers which may affect the discharge planning process. Social Worker supports and educates the interdisciplinary team regarding appropriate community resources and facilitates linkages for continuity of care.
Responsibilities
- Conducts psychosocial assessments and screenings within 72 hours of admission to identify patient/family needs, documenting findings and interventions in the medical record.
- Provides social work interventions including counseling, crisis intervention, advocacy, discharge planning, protective services referrals, and linkage to community resources.
- Coordinates with interdisciplinary teams to develop and implement safe discharge plans for routine and complex cases (e.g., homelessness, lack of insurance, guardianship).
- Serves as a liaison and lead resource for the healthcare team by facilitating communication, multidisciplinary rounds, family conferences, and referrals; provides guidance on high-risk issues such as abuse, psychiatric needs, and chemical dependence.
- Supports patients and families in chronic illness, end-of-life, hospice, and palliative care needs, and delivers education through in-service trainings on relevant psychosocial issues.
- Ensures timely and accurate documentation of psychosocial assessments, treatment plans, interventions, and outcomes, with care delivered appropriately across all patient age groups.
- Schedule: Monday-Friday, 11:00 AM - 7:00 PM (35 hours per week)
- LMSW from an accredited School of Social Work required.
- 2-year prior experience preferred in medical social work or comparable experience/setting/population, depending upon area assigned.
- Knowledge of all rules and regulations of child abuse/neglect reporting as appropriate.
- Documented experience and knowledge of two direct practice modalities.
- Demonstrated ability to use the computer as required by the position.
- Excellent oral and written communication skills.
- Excellent interpersonal skills.
- Good problem-solving, decision-making and judgment skills.
- Must read, write and speak English to the extent required by position; knowledge of a second language preferred and may be required for some positions.
