Job Title: Veterans Directed Care / Options Counselor
Reports To: VDC Manager

General Definition:

Under the regular supervision of the VDC Manager, the Veterans Directed Care Options Counselor provides person-centered counseling services to veterans of all ages at risk of nursing home placement. This position is responsible for assessing, identifying, coordinating, and monitoring supports and services, ensuring compliance with program guidelines, and assisting veterans with financial and employer-related responsibilities. The role also includes monitoring VDC spending in collaboration with the veteran, the Financial Management Service provider, and the Veterans Affairs Medical Center (VAMC).

Essential Duties & Responsibilities ~ Client Services (50%)

  • Conducts initial in-home assessments and periodic reassessments.
  • Develops and monitors individualized spending plans for veterans.
  • Assists veterans in navigating employer responsibilities, including facilitating new hire processes.
  • Provides ongoing training and remediation as needed.
  • Maintains a minimum caseload of 40 veterans, ensuring adherence to program requirements.
  • Performs monthly, quarterly, semi-annual, and annual check-ins as required.

Administrative & Fiscal Responsibilities (50%)

  • Maintains accurate and up-to-date client records in the program database.
  • Tracks expenditures and prepares financial reports.
  • Follows up on referrals and ensures service implementation for clients.
  • Generates and submits required monthly reports.
  • Performs additional duties as assigned.

Knowledge, Skills & Abilities

  • Proficiency in Microsoft Office Suite and standard office equipment.
  • Strong organizational and multitasking abilities.
  • Excellent verbal and written communication skills.
  • Ability to mediate conflicts and facilitate problem-solving.
  • Understanding of client confidentiality and data management.
  • Ability to work independently and maintain schedules.
  • Strong interpersonal skills, with the ability to collaborate with clients, families, agencies, and colleagues.

Education & Experience

  • Bachelor’s degree in Social Work, Gerontology, Community Organization, or a related field preferred.
  • Minimum of five (5) years of progressively responsible experience related to the duties outlined above.
  • College training in a relevant field may substitute for experience at a rate of one year of college for nine months of required experience.

Special Requirements

  • Possession of a valid Virginia driver’s license.
  • Able to drive extended distances, both inside and outside of the Fifth Planning District.
  • Ability to meet physical demands, including in-home assessments and climbing stairs.
