Position Summary

We are seeking a Medical Social Wokers with hospice experience to join our team. The ideal candidate will provide emotional support, counseling, resource navigation, and advocacy to patients and their families. This role is essential in helping patients achieve comfort, dignity, and peace while supporting families through the challenges of serious illness.

Responsibilities

• Drive to patient's and provide support to patients and families
• Reach out to families by phone prior to visits to confirm needs and establish communication
• Provide psychological support and counseling to patients and families
• Assess patient and family needs and develop individualized care plans
• Assist patients and families with advance care planning, end of life decisions, and community resources
• Collaborate with the interdisciplinary team to ensure holistic care
• Support patients and families in navigating emotional, financial, and social challenges related to terminal illness
• Provide crisis intervention and grief counseling when needed
• Maintain accurate and timely documentation in compliance with Medicare regulations and agency standards
• Follow all requirements for visit frequency and visit notes as outlined by hospice regulations
• Demonstrate excellent communication skills with patients, families, and team members

Qualifications

• Active state license LCSW preferred LMSW considered
• Hospice or palliative care experience strongly preferred
• Strong communication and counseling skills
• Ability to work independently and as part of a team
• Compassionate, patient, and professional demeanor

Note
Disciplines are expected to attend interdisciplinary team meetings twice a month. Compensation is per visit and includes completion of visit notes.

Pay: From $90.00 per hour
