SCHEDULE: Mon – Fri, 8am – 5pm EXPERIENCE REQUIRED: 1 year of experience in a healthcare setting; work must have included social work provisions to frail and/or elderly populations. Counseling theory and techniques and ability to effectively and efficiently utilize EPIC EMR and other documentation outlets. Knowledge of community resources and appropriate utilization to meet the needs of clients and employees. Organization and project management skills. Ability to establish and maintain good interpersonal relationships. Ability to work independently and meet deadlines. Comfort performing home visits to private homes of San Diego PACE applicants and participants with frequent travel in San Diego county. EDUCATION REQUIREMENT: Master's Degree from an accredited School of Social Work CERTIFICATIONS REQUIRED: Valid California driver’s license. Reliable transportation with proper insurance to meet San Diego PACE guidelines. First Aid/CPR certification. LANGUAGE REQUIREMENTS: Excellent verbal and written communication
Position Summary
Under the supervision of the Social Services Technical Manager, the PACE Social Services Supervisor oversees the site-specific San Diego PACE Social Services provision. This role ensures quality, efficiency, and accessibility of Social Services to PACE participants and families. The Supervisor leads social workers and case managers, ensuring high standards within the Social Service Department. Responsibilities include carrying a caseload as needed, training social workers and case managers, and overseeing department outcomes. Weekly quality and productivity reports are provided to the Site Manager and Social Services Technical Manager.
Essential Functions of the Job
- Responsible for the provision of direct social work and case management services to PACE participants, adhering to policies, procedures, and applicable laws.
- Ensures active engagement of Social Workers during Interdisciplinary Team meetings.
- Trains social workers to conduct thorough psychosocial assessments, care planning, and periodic re-assessments.
- Participates in hiring, training, and development of Social Workers and case managers.
- Oversees the development of staff, providing feedback and monitoring services.
- Ensures resources are available for appropriate referrals and acts as a liaison with community agencies.
- Consults with staff on social, emotional, and cultural factors impacting health.
- Fosters a professional environment and maintains a cohesive team.
- Participates in program and policy development for the Social Work department.
- Provides clinical and crisis management support.
- Coordinates hospital discharge with the interdisciplinary team.
- Communicates with family, legal representatives, and caregivers as appropriate.
- Acts as a resource for education and training on topics like dementia and difficult behaviors.
- Ensures documentation is accurate, up-to-date, and compliant with regulations.
- Acts as site program administrator in the absence of the Social Service Technical Manager.
- Resolves employee issues within position responsibilities.
Additional Duties and Responsibilities
- May work on special assignments, including traveling to other San Diego PACE sites.

Job RequirementsEducation Required

- Ability to meet standardized competencies for the Social Services Department.

Equipment Used
- Office equipment including computers, phones, copy machines, and fax machines.
Working Conditions and Physical Requirements
- Prolonged periods of sitting and standing, frequent walking, and driving within San Diego County.
- Lifting of no more than 15 lbs.
- Ability to remain calm and objectively evaluate problems.
- May be required to work outside normal working hours.
Universal Requirements
- Pre-employment requirements include I-9, physical, positive background and reference check results, complete application, new hire orientation, and pre-employment PPDs.
Job Type: Full-time
Pay: $39.00 - $50.00 per hour
