Key Responsibilities:

• Lead, develop, and implement departmental policies and goals for Social Services

• Oversee assessments, care planning, and support resident adjustment

• Supervise Social Services team members

• Coordinate discharge planning beginning at admission

• Engage in occasional community outreach initiatives

• Apply a strong working knowledge of relevant concepts, practices, and regulations

• Utilize creativity, experience, and sound judgment to meet departmental objectives

Qualifications:

• Minimum of 2 years’ management experience in a long-term care (LTC) setting

• LSW required; MSW preferred

• Strong knowledge of applicable State and Federal guidelines

• Outstanding interpersonal and leadership abilities
