Overview
We are seeking a dedicated, organized, and detail-oriented Intake Coordinator to join our healthcare team. The Intake Coordinator is responsible for coordinating all new patient admissions, verifying insurance, obtaining physician orders, and ensuring seamless communication between referral sources, clinical staff, and patients. This role plays a vital part in ensuring timely and accurate initiation of home health services.
This position requires strong organizational skills, knowledge of medical terminology, and experience with various electronic health record (EHR) systems.
The Intake Coordinator will serve as a vital link between patients and healthcare providers, facilitating smooth operations within the clinic.
Key Responsibilities
- Receive and process all new referrals from physicians, hospitals, and facilities.
- Verify insurance coverage and benefits; obtain authorizations as required.
- Accurately enter patient demographic and clinical information into the EMR system.
- Communicate with physicians’ offices, referral sources, and interdisciplinary team members to ensure timely and complete documentation.
- Coordinate the scheduling of initial visits with clinical staff.
- Ensure compliance with Medicare, Medicaid, and private insurance requirements.
- Maintain confidentiality and professionalism in all communications.
- Provide excellent customer service to patients, families, and referral partners.

Qualifications
- High school diploma or equivalent required; Associate’s or Bachelor’s degree in healthcare administration or related field preferred.
- Previous experience in home health intake or scheduling strongly preferred.
- Working knowledge of Medicare, Medicaid, and private insurance guidelines.
- Proficiency with EMR systems and Microsoft Office Suite.
- Strong organizational skills with attention to detail and accuracy.
- Excellent verbal and written communication skills.
- Ability to multitask and work in a fast-paced environment.
