Overview:

Sentara Home Care Services is hiring a full time Care Transition Coordinator to support hospice for the Hampton Roads area.

Schedule: Fulltime, 8am-5pm, M-F

Location: Sentara Princess Anne Hospital in Virginia Beach

Overview

Responsible for the identification and assessment of home care and hospice needs of patient population and coordination of these services to ensure a smooth transition to the next site of care.

Certification/Licensure

  • Licensed RN, LPN, RT or BSW required. Proof of education as LPN, RN, RT or BSW required.


Experience

  • 1 year of home care, discharge planning or case management experience required.
