BHN is currently seeking an Outreach Counselor to join our Community Support Program (CSP), supporting individuals with behavioral health and substance use needs in the community.

What You’ll Do
As an Outreach Counselor, you will provide person-centered, in-person, and telehealth services to individuals in the community to help them overcome barriers and connect to needed supports.
- Meet individuals in hospitals, treatment facilities, and community settings to assist with discharge planning and continuity of care
- Provide care management and support for individuals with behavioral health and substance use needs
- Act as the primary point of contact for your caseload, ensuring needs are identified and met
- Complete assessments, individualized action plans, crisis plans, and discharge plans in collaboration with providers and natural supports
- Educate and connect individuals to community resources including housing, food assistance, healthcare, employment, and legal supports
- Maintain collaborative relationships with community providers and stakeholders
- Document services in a timely and professional manner
- Support individuals in developing coping skills, independence, and self-sufficiency
Who You Are
- High School Diploma or GED required; Bachelor’s degree or Community Health Worker certification preferred
- 1–2 years of experience in behavioral health, substance use, corrections, or related human services field required
- Knowledge of mental health, substance use, and community resource navigation
- Experience working with justice-involved populations preferred
- Bilingual (Spanish/English) strongly preferred
- Strong communication, interpersonal, and time management skills
- Valid Massachusetts driver’s license and reliable vehicle required
