About Job

Care Managers provide outreach and enrollment services to adults presumed to meet eligibility requirements of New York State’s Health Home program. They gather assessments and enter documentation that confirms eligibility and identifies areas requiring supportive services. They work with consumers and service providers to create a comprehensive Plan of Care and coordinate the continuing involvement of supports in the execution of the plan.

Principal Activities

  • Doing a comprehensive annual assessment.
  • Developing a Plan of Care for individuals annually.
  • Develop a Crisis Plan annually.
  • Coordinate care for the consumer with members of the care team.
  • Ensure that consumers have all needs met including food, transportation, housing, etc.
  • Document all activity in an accurate, organized filing and tracking system.
  • Assures quality service delivery by monitoring program activities, records, data filing, and summarizing information.
  • Maintains professional and technical knowledge by attending educational meetings, training, and workshops.
  • Promotes a positive agency image and creditability by working with administration on funding sources and other community services; adhering to a professional code of ethics.
  • Providing transportation, as necessary.
  • Contributes to team effort by accomplishing related results as needed or as requested by supervisor and/or Executive Director.
  • Demonstrates sensitivity towards consumers with disabilities.
  • Knowledge of area service providers and their programs.
  • Contributes to team effort by accomplishing related results as needed or requested by supervisor and/or Executive Directors.

Job Qualifications

  • 4-year college degree in Human Services prefered and experience dealing with disability related issues.
  • Experience in a Human Service environment.
  • Communicates well verbally and in writing.
  • Proficient in Microsoft Office.
  • Maintains a clean and safe working environment.
  • Effectively manages multiple tasks and priorities
  • Understands and performs job results in a non-judgmental manner.
  • Ability to interpret policies, procedures, and other written materials.
  • Accepts supervision and follows through with supervisor recommendations.
  • Treats colleagues, staff, and program participants with respect.
  • Must be able to comply with HIPAA requirements.
  • Interpersonal communication skills
  • A valid driver's license is required.

Job Type: Full-time

Pay: $20.00 per hour
