Job Summary:

Affinity Healthcare San Diego, Inc., DBA Art of Care, is seeking a highly motivated and organized Care Coordinator to join our growing team. This is an immediate-start, per diem position with the potential to transition to part-time and eventually full-time employment. Once full-time, benefits will be offered.The Care Coordinator will support the Home Health Administrator by assisting with care coordination for clients and performing select human resources duties, including onboarding and assisting new employees. This role requires a professional, compassionate individual who can thrive in a fast-paced healthcare environment while maintaining compliance with company and regulatory standards.

Key Responsibilities:

Assist the Home Health Administrator with day-to-day administrative and human resources duties.

Support onboarding processes for new employees, including orientation and documentation.

Serve as a point of contact for patients, caregivers, and staff regarding scheduling, care coordination, and administrative matters.

Ensure proper documentation of care plans, patient records, and HR-related files in compliance with company policies and regulatory requirements.

Maintain clear and professional communication with internal and external stakeholders.

Participate in team meetings and assist with other administrative projects as assigned

Qualifications:

Proven experience in care coordination, healthcare administration, or human resources is preferred.

Excellent organizational, communication, and interpersonal skills.

Ability to work independently while supporting a collaborative team environment.

Strong attention to detail and ability to manage multiple priorities.
