Expected pay for this position is $31.49 - $37.05/hour. Actual pay will be determined by experience, skills and internal equity. This is an Hourly position. Overview:
The Case Manager, BHS Consult Liaison is responsible for identifying, assessing, planning, coordinating, implementing, monitoring and evaluating options and services for the patient with acute mental health symptoms across the continuum of care for individuals in assigned case load. Offers impactful collaboration, and when directed by the attending physician coordinates services and the health care team to facilitate and expedite the best outcomes and satisfaction with services for the patient and physician caring for that patient. Works with patient and treatment team to help establish goals and realize optimal outcomes as determined by current individualized care plans. Demonstrates the knowledge and skills necessary to provide services to the assigned patient population. Qualifications:
Bachelor's degree in Social work or other related mental health field
Licenses/Certifications:
Clinical Counselor License
Experience:
Other skills/knowledge:
Excellent interpersonal and communication skills.
Solid computer skills, including proficiency with Microsoft software.
Strong analytical and problem solving skills, with the ability to be detail oriented.

REQUIRED QUALIFICATIONS:
Education:
Bachelor's degree in Social work or other related mental health field
Licenses/Certifications:
Must have one of the following Licensures:
Clinical Counselor License
or
Clinical Social Worker License
or
Professional Counselor License
or
Social Worker License
Experience:
2 years experience in behavioral health care management or related field
Other skills/knowledge:
