We are seeking a compassionate and experienced Social Worker to join our dedicated team at a skilled nursing and rehabilitation facility. This role is essential in supporting residents and their families as they navigate the emotional, social, and practical aspects of long-term care.

Responsibilities:

• Provide psychosocial support and counseling to residents and their families

• Assist with admissions, discharges, and care planning processes

• Participate in interdisciplinary team meetings and help develop individualized care plans

• Serve as a liaison between residents, families, facility staff, and community resources

• Address concerns related to resident rights, satisfaction, and quality of life

• Maintain accurate documentation in compliance with federal and state regulations

Qualifications:

• Minimum of 3 years of experience as a Social Worker in a long-term care, hospital, or related healthcare setting

• Bachelor’s degree in Social Work or related field preffered but not required

• Active Social Work license preferred, but not required

• Strong communication, organizational, and problem-solving skills

• Ability to work collaboratively as part of a multidisciplinary care team
