Position Summary
We are currently seeking a PACE Activity Coordinator to join our Care Coordination / PCMH team at Alpine, CA. This is a temporary, full-time position for a 19-week assignment, starting September 22, 2025.

Title: PACE Activity Coordinator
Location: Alpine, CA 91901
Duration: Temporary | Full-time | Variable Shift

Key Responsibilities

• Plan, coordinate, and implement recreational and therapeutic activities for PACE participants
• Collaborate with interdisciplinary care teams to assess participant needs and preferences
• Maintain accurate documentation of attendance and progress notes
• Support wellness, social interaction, and cognitive stimulation among participants
• Adhere to infection control, safety, and compliance guidelines

Preferred:

• Previous experience working with PACE or geriatric populations
• Knowledge of activity planning and group facilitation techniques
• Bilingual (Spanish/English) is a plus

Job Types: Full-time, Contract

Pay: $24.00 - $27.00 per hour

Expected hours: 40 per week

Experience:

• Senior care: 1 year (Preferred)

License/Certification:

• BLS Certification (Preferred)
