Job Summary: The TCOOMMI Case Manager supports clients who are transitioning into the community after incarceration. This position completes screenings, biopsychosocial/intake assessments, treatment plans, service coordination/case management, medication training and support and living skills training. This position is primarily clinic-based with required flexibility for community engagements, when clinically appropriate. This position collaborates closely with criminal justice partners, including TDCJ Parole, Probation and/or Pretrial for treatment planning and coordination of services. This position works closely with interdisciplinary teams to provide comprehensive care to clients. This position reports to the Practice Manager and may provide support with completion of program reports, documentation reviews, internal compliance audits and leading of clinical staffing/meetings.
Qualified Mental Health Professionals (QMHP) embrace trauma-sensitive and informed practices, provide Suicide Safer care, and foster a supportive and welcoming environment. QMHPs operate in a dynamic environment, develop extensive knowledge of community resources, collaborate with interdisciplinary teams, and cultivate a clinical skill set by forging therapeutic relationships with community members and/or stakeholders. Interventions and services can be both office- and community-based and may require the use of a personal automobile. Individuals served may be diagnosed with mental illness, co-occurring substance use disorders, chronic health conditions, and/or IDD.
Staff members in this position are working under the supervision of a clinically licensed professional who provides ongoing evaluation and oversight of all assessments, diagnoses and treatment. At this level, clinical assignments may be of moderate complexity and are handled with considerable independence, and complicated cases receive closer clinical review and supervision.
Services provided may include individual, family and/or group skills training using evidence-based interventions, crisis assessment, and case management.
Work is performed under general supervision with moderate latitude for the use of initiative and independent judgment.
Regular, predictable attendance is considered to be an essential requirement for this position. Other duties may be assigned. May require a flexible work schedule.

Workdays: Tuesday - Friday.
Workhours: 7:30AM - 5:30PM or 8:00AM - 6:00PM.
Salary: $24.17 - $25.38 an hour based on education.

Education
- Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Human/Behavioral/Social Services or related field required. Masters Degree preferred.
- Current and valid driver's license and a clean driving record.
