Position Summary:
Under the supervision of the PACE Home Care Coordination RN Manager and as a member of the Interdisciplinary Team (IDT), the Home Care Coordinator (HHA) provides support in coordinating unskilled home-based care for San Diego PACE participants. This includes conducting home safety evaluations, coordinating IDT-approved services (meals, caregiver support, DME), and advocating for participants' healthcare preferences in a person-centered and compassionate manner.
Essential Job Functions:
Coordinate delivery of IDT-approved services: meals, DME, and caregiver support.
Complete care plans at enrollment and semi-annually.
Perform pre-enrollment, annual, and change-of-address home safety evaluations.
Document services, updates, and progress in EMR systems per DHCS and CMS guidelines.
Monitor service quality and coordinate adjustments with care providers.
Conduct and document monthly visits for homebound and RCFE participants.
Assist nurses with supply/equipment delivery and participant support.
Participate in IDT meetings, trainings, and ensure infection control measures.
Minimum Qualifications:
Education:
Certified Nursing Assistant (CNA)
California Department of Public Health Home Health Aide (HHA) Certificate
CPR and First Aid Certification (American Heart Association)
Experience:
At least one year of experience working with frail or elderly populations
Valid California Driver's License
Technical Skills:
Basic computer skills; experience with Microsoft Outlook and Excel
Ability to use EMR systems and maintain accurate documentation
Additional Requirements:
Must operate within Home Health Aide scope of practice.
Strong communication skills; collaborative team player.
Compliance with HIPAA, OSHA, PPD testing, vaccinations, and all San Ysidro Health policies.
Participation in sexual harassment, OSHA, and customer service trainings.
Pre-employment clearance including background check, physical, and I-9.
