Major/Essential Functions
Maintain current information on services and products needed by persons with disabilities and be available to conduct presentations as needed.
Participates in Departmental Performance Improvement which will satisfy both institutional and contractual requirements
Provide phone coverage.
Employee is responsible for convening service planning meetings.
Employee is responsible for completing face to face quarterly review.
Employee Responsible for communicating with Direct Service Agency and Individual/Family.
Collaborate with collateral agencies and providers to ensure compliance with Individual Service Plan.
Be on call on a rotating basis and counsel with clients and provide crisis intervention and advocate as needed
Conduct home visits as needed.
Perform CLASS billing monthly.
Obtain all necessary authorization to provide services.
Performs all other related duties, within the scope of practice, on an as needed basis or as required by Supervisor. Assist with planning, coordination and follow up on WTCMP special projects and provide CLASS training to new employees.
Adhere to all TTUHSCEP policies, procedures and processes.
Personally demonstrate, display, and act in accordance with TTUHSCEP's values (Service, Respect, Accountability, Integrity, Advancement, and Teamwork). Actively promote and encourage staff across the institution.
Remains current with all licensure, certifications and mandatory compliances and trainings required of this position.
Perform all other duties as assigned.
Required Qualifications
Bachelors degree in social work from an accredited college or university.
Must be licensed by the Texas State Board of Social Work Examiners within 6 months of start date.
Licensure/certification
Licensed by the Texas State Board of Social Worker Examiners.
Preferred Qualifications
Individual casework practice, crisis intervention, information and referral, case management and interviewing.
Experience working with persons with disabilities.
Knowledge of Social work process, ability to assess client's needs, ability to compile/maintain information on community resources.
Ability to develop positive relationships with clients and collateral agencies.
Problem solving and decision making skills.
Ability to conduct a client interview and ability to use a personal computer.
