**Responsibilities:**
- Coordinate patient care activities to ensure appropriate follow-up and continuity of care
- Schedule patient appointments and maintain accurate patient records
- Communicate with healthcare providers, patients, and insurance companies
- Assist in managing referrals to specialists
- Provide administrative support.
- Must be a team player
**Experience:**
- Previous experience in an office environment, preferably in home health or hospice office
- Strong organizational skills and attention to detail
- Excellent communication and interpersonal abilities
