POSITION DESCRIPTION: The Client Care Coordinator, with leadership from the Social Service Supervisor, is responsible for the following goals: (1) enhance the delivery and coordination of mental health and related services to families with children in shelter, (2) promote and model best practices for shelter social service staff; (3) improve linkages to mental health and community based services; (4) increase the ability of shelter social service staff to address mental health issues in culturally and linguistically sensitive manner that incorporates strength-based, family-driven and youth/child-guided care and (5) strengthen overall permanency outcomes for families with children.
REPORTS TO: Social Service Supervisor
Client/Service Delivery
· Completes a comprehensive bio psychosocial assessment with each family to understand strengths and services needs within 30 days;
· Facilitates a team review/discussion of each biopsychosocial assessment;
· Makes referrals and appropriate linkages and helps determine the need for any further intervention;
· Collaborates with ACS and/or preventive in ACS conferences to advocate for the family;
· Provides short-term counseling on and assistance with specific stressors associated with living in shelter;
· Provides crisis intervention when needed in conjunction with rest of social service team;
· Facilitates psycho-education, groups, workshops, trainings, and other evidence-based approaches to engagement;
· Conducts risk assessments and coordinates proactive and required safety planning;
· Prepares families and children to be receptive to accepting services and necessary interventions;
· Communicate and collaborate with service providers to enhance engagement and ensure quality;
· Provides quarterly education and training sessions/information for staff to facilitate holistic approaches to care;
· Facilitates quarterly family meetings (with family + shelter staff) to discuss planning efforts and progress;
· Works from a strength–based, family-focused, client-centered perspective to identify and fortify family strengths;
· Ensures that families are receiving full support via internal assistance and external refers for services;
· Confers and consults with professional and technical personnel implementing and multidisciplinary approach to client care and well-being;
· Develops linkage agreements with community-based organizations, including local clinics in shelter catchment areas;
· Serves as an agency and program liaison to community agencies and/or groups involving various stakeholders;
Team/Staff Relations
· Work as a Providence House team member with all staff to implement the mission and core values of the agency;
· Attend regular internal and external agency meetings;
· Work as part of a team to plan and develop resident programs.
Administrative
· Maintains timely and accurate documentation on all clients (ex: treatment plans, SOAP notes, case conference notes, workshop summaries, engagement attendance logs, family meeting agreements, etc.)
· Completes all paperwork in a swift and detailed manner that ensures payment to the agency from various sources;
· Engages in data entry across multiple software systems preparing relevant reports, and assists data responsibilities;
· Performs any other mission-relevant duties as assigned by Social Service Supervisor and/or program leadership.
Sanctuary Model
· Effectively use and practice Sanctuary Models tools; safety plans, community meetings, psycho-educational groups, team meetings, red flags reviews, and self-care in the program to create a non-violent and trauma sensitive culture.
· Promote and model the core values of Providence House Mission (Compassion, Non-Violence, Hospitality, and Community) and the Seven Sanctuary Model Commitments to create a non-violent and trauma sensitive culture.
· Integrate the Sanctuary Model SELF (Safety, Emotional Management, Loss and Future) framework into daily practice to ensure quality care and an effective work environment.
REQUIREMENTS:
· Licensed Master’s Social Worker (LMSW)
· Must obtain license within 3 months of hire.
· Two (2) years of experience working with vulnerable populations.
· Knowledge of child & adolescent development, parent child relationships.
· Strong expertise in strengths based, solution focused, family centered practice.
· Excellent verbal and written communication skills and the ability to work independently.
· Excellent interpersonal and organizational skills
· Competent technology & management skills, including proficiency with using Microsoft Office; and client database management.
WORK SCHEDULE: The Client Care Coordinator position is a full-time, salaried exempt position. Generally, the hours worked will be weekdays from 9:00a.m. – 5:00p.m., with one weekday agreed upon at hire as a “late day” with an adjusted schedule starting later in the day and ending later to allow for time to assist clients who may be working during the day. This role will require occasional evening and/or weekend work beyond the routinely scheduled “late date” with and without advance notice. The agency has headquarters in Brooklyn, New York and this position is based at 820 Glenmore Ave in Brooklyn, New York.
SALARY AND BENEFITS: The starting salary for this salary, exempt position is $70,000 annually. Paid time off benefits include: 14 paid holidays, 3 weeks of vacation (with additional accruals based on agency tenure), 4 personal days (1 in recognition of your Birthday), 12 sick days, and early closure on Fridays proceeding Monday holidays.
