Description:

Notre Dame Health Care's Pediatric Palliative Care program (Pedi Pals) has been caring for children with life limiting-illnesses since 2012, with the goal of improving quality of life. The Pedi Pals program serves the unmet physical, emotional, social, and spiritual needs of eligible children and their families in Massachusetts


The primary purpose of this position is to plan, organize, develop, and direct all required social services to meet the needs of residents in accordance with federal, state, and local standards, guidelines and regulations, that govern our facility, and as may be directed by the Director to assure that the medically related emotional and social needs of the resident are met/maintained on an individual basis.



Responsibilities

  • Follow and maintain HIPAA guidelines and regulations with respect to all patient information. Protect the security of medical records to ensure that confidentiality is maintained.
  • Interact with Managers and/or Directors on a professional level to ensure administrative needs are being met.
  • Responsible for the planning, coordinating, providing and documenting care for terminally ill patients and their family members from day of admission through day of discharge.
  • Develops, updates, and implements a Pedi pals care plan that meets each patient's specific needs and is in compliance with federal and state regulations, reimbursement guidelines as well as the organization's policies and procedures.
  • Managing frequencies of the interdisciplinary team as it pertains to the pediatric patient's plan of care.
  • Actively participates in interdisciplinary team meetings and able to deliver strong case presentations.
  • Actively participates in staff and community meetings.


Requirements:
  • Graduate of an accredited school of Social Work with current licensure in the state of Massachusetts
  • Strong background in mental health/counseling and experience in health services delivery
  • Ability to work with other members of the Interdisciplinary Team
  • Ability to conduct assessment of patient/family psychosocial history
  • Ability to identify and prioritize problems with appropriate goal formulation and conduct required interventions
  • Demonstrated leadership capabilities with the ability to relate well to staff and other professionals
  • Valid state driver's license and reliable automobile with proof of current automobile insurance
  • Expected to participate in appropriate continuing education as required to maintain licensure
  • Possesses excellent verbal and written communication skills
