Essential Functions
- Develops a comprehensive social history and psychosocial assessment that includes the patient's problems and strengths and preferences, including cultural, religious, and ethnic background and implications for the care plan.
- Helps patients and their families (in their social, racial, ethnic and cultural context) cope with the immediate effects of the decision to be admitted to hospice care.
- Orients patients and families to the hospice, its services, its service limitations and patients' rights.
- Involves patients and families in end of life planning.
- Attends scheduled ITM meetings and identifies and interprets psychosocial needs of patients for inclusion in the care plan, addressing in particular issues of schedule, treatment and environment that express and reinforce individuality and identity.
- Completes mandatory documentation within timelines established by applicable regulations.
- Empowers patients and encourages patient choice in matters affecting them, demonstrating receptive attitudes and sensitivity to choices based on racial, cultural and ethnic heritage, and sexual orientation.
- Mediates issues that arise among patients, families and staff.
- Ameliorates emotional distress of patients and families.
- Provides linkage with appropriate community resources by maintaining knowledge of other systems, making referrals and identifying unmet needs (e.g., recreational transportation, adaptive phone equipment, etc.)
- Provides patients and families with crisis management services.
- Assists patients and families with financial questions and makes appropriate referrals.
- Coordinates visits by facility mental health providers and makes patient referrals.
- Assists endowment patients with managing financial concerns (e.g., business mail, billings, Medical Assistance spend downs, prepaid funerals, etc.)
- Helps patients and family prepare for and cope with losses, including aging and death. Provides Advance Directive Information and forms.
- Trains and consults with staff regarding psychosocial needs that encompass social, ethnic, religious and cultural diversity of individual patients, the patient group as a whole and families to increase staff sensitivity.
- Educates staff about the Patient Bill of Rights and any relevant state legislation.
- Advocates on a case, policy and program level.
- Participates in policy decision making in a way that encompasses an understanding of the issues and implications relevant to religious, ethnic and cultural diversity as it affects patient life and care and family involvement.

Supervisory Responsibility
This position has no supervisory responsibilities.
Work Environment
This job operates in a professional office environment and in the homes of our patients. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.
Physical Demands
While performing the duties of this job, the employee is regularly required to talk or hear. The employee frequently is required to stand; walk; use hands to finger, handle or feel; and reach with hands and arms. The employee is required to be able to drive oneself to and from patient homes.
Position Type and Expected Hours of Work
This is a contracted temporary full-time position, and hours of work and days are Monday through Friday, 8:00 a.m. to 4:30 p.m. Occasional evening and weekend work may be required as job duties demand.
Travel
Travel is primarily within a radius of 50 miles from our main office. Frequent travel is required.
Required Education and Experience
- Bachelor’s degree in social work from an accredited school of social work.
- Licensed by the State Board of Social Work.
Preferred Education and Experience
- Two (2) years of supervised social work experience.
Additional Eligibility Qualifications
- Must be able to function independently and have flexibility, personal integrity and the ability to work effectively with residents, staff and support agencies.
- Must be able to relate to and work with ill, disabled, elderly, emotionally upset and at times hostile people within the facility.
- Must possess the ability to make independent decisions when circumstances warrant such action.
- Must be able to pass an extensive background check and drug screening.
- Must be insured by an auto insurance provider in accordance with state law and must possess valid driver license.
