Key Responsibilities
As a Hospice Social Worker, you will provide a wide range of social work services to patients and families, including emotional, spiritual, and psychosocial support. You will play an integral role on the Hospice IDG (Interdisciplinary Group) by:
Performing initial assessments and assisting in care plan development
Conducting ongoing reassessments and providing counseling as needed
Offering short-term crisis intervention and family counseling
Supporting patients and families through issues of death and dying
Serving as an advocate for the patient/family unit
Participating in bereavement services and community outreach
Coordinating with community agencies and referral sources
Documenting visits, assessments, and interventions clearly and on time
Additional Expectations
Attend staff meetings, IDG conferences, and trainings as assigned
Participate in orientation and ongoing professional development
Adhere to Hospice policies and maintain professional standards
Exhibit the Hospice philosophy in all interactions and care delivery
Complete clinical notes for all services provided within 48 hours
- Maintains the agency's mission, philosophy, and core values.
- Ensures compliance with agency policies and procedures regarding operations/processes, including but not limited to those regarding patient care, patient complaints, incidents, safety, and emergency management.
- Always maintains patient confidentiality, including all HIPAA regulations.
Regulatory Requirements
- Licensed or registered in the State of Michigan.
- Must pass a criminal background check & MVR check.
- Completed health statements acknowledging the ability to perform the duties of the position.
- Valid state drivers license.
- Proof of current automobile insurance.
- TB testing per agency policy;
Industry experience
- Ability to work independently, make accurate, and at times, quick judgments. Ability to respond appropriately to crises outside of a hospital setting. Acceptance of and adaptability to different social, racial, cultural, and religious modes.
- Minimum 2 years of experience as a social worker, preferred. Active patient contact within the past three years is preferred.
EDUCATION:
- MSW Degree from a school of social work accredited by the Council on Social Work Education (CSWE), and one year of experience in a health care setting.
- Baccalaureate Degree in social work (BSW) from a school of social work accredited by the CSWE, and one year of experience in a health care setting and be supervised by an MSW from a school of social work accredited by the CSWE and who has one year of experience in a health care setting. If the BSW is employed by The Care Team before December 2, 2008, he/she is exempted from the MSW supervision requirement.
- Have a Baccalaureate Degree in psychology, sociology, or other field related to social work, and at least one year of social work experience in a health care setting and be supervised by an MSW from a school of social work accredited by the CSWE and who has one year of experience in a health care setting.
