Salary: $30,000 - $49,000 per year (commensurate with experience)

Job Summary

We are seeking a compassionate and organized Human Service Coordinator to join our team. This role involves direct client support (case management), administrative responsibilities, and community outreach. Responsibilities

• Conduct client orientation, assessments, and discharge planning (onsite and offsite).
• Assist individuals in finding employment and permanent housing.
• Obtain and manage releases of information and referrals to community-based agencies.
• Maintain and update resident files with accurate documentation.
• Coordinate and schedule use of conference rooms.
• Track and analyze data using Microsoft Office.
• Collaborate with owners to enhance residents' experiences.
• Provide education on NARCAN use and distribute supplies as needed.
• Transport individuals to local food banks, grocery stores, and workplaces.
• Promote infection prevention practices and educate residents.
• Clean and disinfect resident rooms during turnover periods.
• Conduct room inspections for cleanliness and safety.

Required Qualifications

• Strong problem-solving and critical-thinking skills.
• Ability to effectively manage crises with sensitivity and empathy.
• Experience in human services, organizational management, program development, or adult education and training.
• Commitment to confidentiality and neutrality, with a focus on sensitivity and empathy.
• Proficiency in Microsoft Office.
• Valid NYS driver’s license with excellent driving skills.

Preferred Qualifications

• Bachelor’s degree or significant experience in human services, social work, or case management.
• Familiarity with local community agencies and resources.
• Experience working with interns.
• Knowledge of substances uses disorders and mental health conditions.
• Certification in NARCAN use.
• First Aid certification.
