Position Overview:

Sound Wellness Center is seeking a compassionate, experienced, and dedicated Psychiatric Rehabilitation Program (PRP) Counselor to join our team on a contractual basis. This hybrid position is ideal for a mental health professional who thrives in a flexible work environment while supporting adults facing psychiatric and emotional challenges.

As a PRP Adult Counselor (Service Coordinator), you will provide a combination of in-person, in-office, and telehealth services to ensure clients receive accessible, holistic, and individualized care.

You will play a vital role in helping clients improve their quality of life through assessments, supportive counseling, resource connection, and the development and implementation of individualized rehabilitation plans.

Key Responsibilities:

• Update existing CSSRs and Safety Plans for assigned clients.
• Review and revise initial assessments and IRP treatment plans; align all encounter visits with individualized goals.
• Provide ongoing supportive counseling and coordinate care with the multidisciplinary team.
• Ensure accurate, detailed encounter notes and submit a monthly summary for each client.
• Collaborate closely with clinical teams to deliver integrated and comprehensive care.
• Maintain documentation in compliance with clinic protocols and regulatory standards.

Qualifications & Skills:

• Prior experience working in a PRP setting (adult population preferred)
• Strong proficiency with Google Workspace and built-in EHR systems
• Understanding of ICD coding criteria for PRP client enrollment
• Skilled in clinical assessment, counseling techniques, and treatment planning
• Comfortable using telehealth platforms for remote services
• Excellent documentation and organizational skills
• Ability to maintain confidentiality and work collaboratively in a clinical setting
