Crawford Care Center is seeking a part-time Social Worker who has a love for working with seniors and a passion for healthcare.

Responsibilities include but are not limited to:

• Maintaining and/or improving resident’s ability to control everyday physical needs and mental/psychosocial needs
• Identifying individual social and emotional needs
• Developing and maintaining individualized care plans
• Participating in the planning of the resident’s admission, community re-entry or transfer to another facility
• Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident
• Encourage the resident/family to participate in the development process and review of resident's plan of care
• Report any issues/changes to the Director of Nursing Services
• Ensure that all social services components of the MDS reports are completed and signed in the most efficient manner
• Review nurse's documentation to determine if the care plan in place is being followed properly
• Knowledge of regulatory procedures
• Work with the finance team to ensure resident is educated and the proper documents are in place for the resident during their stay
• Work with the resident on discharge and get necessary DME or home care in place so that the resident is safe when they return to the community

Qualifications/Requirements:

• BSW or Bachelors degree in related field
• At least one year experience in Social Work field required
• Experience in long term care and/or sub-acute setting preferred
