Job Overview
We are seeking a compassionate and dedicated Social Worker to join our team. The ideal candidate will have a strong background in social work, with experience in various settings including hospitals, outpatient clinics, and community care environments. This role involves providing support and counseling to individuals and families, conducting assessments, and developing treatment plans to address mental health and social needs.

Responsibilities

  • Conduct comprehensive patient assessments to identify needs and develop individualized care plans.
  • Provide individual and group counseling sessions focusing on mental health issues, behavioral therapy, and crisis intervention.
  • Collaborate with multidisciplinary teams to ensure holistic patient care, including case management and coordination of services.
  • Maintain accurate medical documentation in compliance with HIPAA regulations, including intake forms and progress notes.
  • Engage in clinical research to enhance service delivery and improve patient outcomes.
  • Facilitate group therapy sessions for various populations, including children, seniors, and individuals with developmental disabilities.
  • Offer support in hospice care settings, providing emotional support to patients and their families during end-of-life transitions.
  • Utilize EMR systems for effective data collection and management of medical records.
  • Provide education on resources available for addiction counseling, PTSD care, Alzheimer’s care, dementia care, and other behavioral health issues.
  • Participate in ongoing professional development to stay current with best practices in social work.

Qualifications

  • Master’s degree in Social Work (MSW) from an accredited institution is required.
  • Valid state licensure as a Social Worker (LCSW or equivalent).
  • Experience in child & family counseling, clinical counseling, or working with special populations such as geriatrics or pediatrics is preferred.
  • Familiarity with medical terminology, ICD coding (ICD-9/ICD-10), CPT coding, and EMR systems is highly desirable.
  • Strong skills in crisis management, motivational interviewing, diagnostic evaluation, and psychotherapy techniques such as cognitive behavioral therapy (CBT).
  • Ability to work collaboratively within a team-oriented environment while maintaining a client-centered approach.
  • Excellent communication skills with the ability to engage effectively with diverse populations.
