Homewatch CareGivers is seeking a Client Care Coordinator at our Ventura, CA location. Homewatch CareGivers provides personalized in-home care to support our clients’ unique needs. As the Client Care Coordinator, you will manage client services, provide supervision and annual reviews for all home care service staff.

Responsibilities:
· Develop, supervise, and evaluate a group of qualified caregivers
· Ensure standards are adhered to through client reviews, caregiver reviews, and training/education
· Client, caregiver, and outside care collaborator relations
· Direct care quality assurance, compliance, and supervision
· Utilize Word, Excel, and other required software
· Assist the President with establishing policies and procedures in compliance with state health statures and administrative rules to include:
o Guidelines for employee safety
o Standard precautions
o Infection control procedures
· Inform appropriate staff about home care statutes and administrative rules
· Coordinating client care services for an assigned group of clients
· Assist in implementing a Quality Improvement Program to ensure achievement of agency objectives
· Maintain confidentiality of agency, employee, and client matters
· Assist with orientation of new staff prior to client contact
· Submit documentation within required timeframe
· On-call responsibilities
· Other duties as needed
Qualifications:
· 2+ years experience in the delivery of healthcare within the last five years
· Excellent problem-solving techniques and abilities
· Knowledgeable of the Federal Conditions of Participation and the statutory and administrative rules of the state’s regulatory bodies, to ensure compliance within the agency
· Communicate clearly and effectively to staff
· Microsoft Word, Excel and/or related experience
· Ability to pass background check
