Description

Working with medical providers to coordinate all aspects of the Medication Only program and necessary documentation and chart maintenance for all medical providers. Ability to multitask and prioritize is a must. Adherence to HIPAA regulations a must.

Responsibilities:

• Assist with obtaining paperwork on clients-labs, physicals, discharge summaries as needed.

• Update and maintain patients’ health record.

• Assist with obtaining consents from clients.

• Use Electronic Health Records to support all transactions.

• Answer patient's questions and ensure quality customer service.

• Collaborate with doctors and nurses to help with obtaining necessary paperwork for appointments.

• Use data resouces for tracking chart completion.

• Communicating with client pharmacies and Primary Care Providers.

• Coordinates the transition of clients from the OPC Medication Only Program and from the Med Only back to OPC.

• Send Med Only letters to newly enrolled clients.

• Check on Med Only clients monthly (if no med apt).

• Keep an updated census for all clinics for the clients enrolled in the Medication Only Program.

• Keep track of the status of Med Only Treatments Plans.

• Send reminder emails of TPR due dates to the medical providers.

• Keep track of discharge summaries.

• Send closing letters and discharge summaries to clients.

• Keep track of safety plans and HQA’s and send status to clinic nurses.

• Check on High Risk cases.

• Keep track of the documentation/consents for the Med Only clients.

• Work with Nurses to ensure medication refills happen smoothly.

• Set up follow-up appointments with medical providers.

Requirements

  • High School Diploma of GED.

  • Office experience preferred

Salary Description

$43,388.80/year
