About Job

This position would be a full-time position located primarily in Wayne County. This is a grant-funded position. This position would report directly to the Lead Case Manager and would work with all clinical and non-clinical staff to provide mentoring, peer support, linkage, and care coordination as needed.

Essential Functions:

  • Provides leadership in development and implementation of early intervention support such as anger management, parenting and budgeting.
  • Assist clients to achieve wellness, stability, and autonomy through access to resources such as housing, employment, and other support services.
  • Facilitate multiple care aspects (case coordination, information sharing, etc).
  • Assist clients with obtaining a GED as needed or other skilled training for employment stability.
  • Develop effective working relations and cooperate with clinical team throughout the entire case management process.
  • Assist with initial intake processes to determine client needs and prioritize through development of effective service plans.
  • Take the extra mile and interact with clients to keep track of their progress and to ensure satisfaction by maintaining regular contact.
  • Record case information, complete accurately all necessary forms and produce statistical reports.

High school diploma or GED, some related work experience is preferred, must be a self-identified recipient of substance abuse services with a history of involvement in the criminal justice system. As a peer in the recovery, there must be at least two years of consistent recovery. Graduate of a recognized Peer Mentor training program such as a Certified Peer Specialist (CPS) with the Georgia CPS Project or Georgia CARES Project, Valid Georgia driver’s license and a reliable form of transportation.
