Summary
The RN Care Coordinator (CC) is charged with an intense focus on improved patient outcome for chronic kidney disease and better patient engagement in the Carolina Kidney Partners (CKP) value-based system. Our goal is to improve the quality of life for patients that have chronic kidney disease. The RN will provide patient care coordination in a clinical role working directly in a nephrology clinic practice . All directives for the role and responsibilities will be provided and managed through CKP leadership team.

CKP Care Coordinator RN will report directly to the CKP RN Manager and CMO.

Data Analytics

The CC will coordinate care utilizing the CKP analytic system, and nephrology providers to implement clinically meaningful improvement in patient outcomes. This data will then be used to measure quality outcomes, population costs, and patient satisfaction. The CC will provide feedback to the medical groups, providers, and company (CKP). The CC will contribute to action loops to implement change in individual patient care, population care, and cost evaluation.

RN Care Coordination (CC)

The RN CC will by provide chronic kidney disease education (treatment modalities including kidney transplant referrals, home or in-center dialysis education), and work to reduce hospital admissions/readmissions. The RN CC will devote 80% of effort in-person with patients in clinic and 20% through telephonic patient communication. We will embed the RN care coordinator in an individual nephrology practice to use the support system that is developed and provide a direct touch for patient centered care.

The care coordinator goals:

  • Contribute to development of dialysis education
  • Contribute to development transplant education
  • Responsibility for optimal dialysis starts metrics
  • Responsibility for CMMI metric collection
  • Responsibility for the development of transitional care, and end of life counseling referrals
  • The RN care coordinator will contribute ideas to policies and procedures for the flow of information

Roles & Responsibilites:

  • Dialysis education with an emphasis on home dialysis and transplant
  • Transplant education and referrals
  • Optimal dialysis start coordination for all patients expected to need a dialysis start
  • Provide treatment modality education for chronic kidney disease patients with stage 4 and 5.
  • Prevent or reduce hospital admissions and discharges
  • Individualize care coordination of patients with collaboration and direction with primary nephrologist
  • Responsibility for CMMI metrics that impact quality
  • Collaborate with the CKP clinical care navigators also embedded in the practice

Education/Discipline Requirements

  • Registered Nurse (RN) License & BLS
  • Experience required in at least one area for kidney disease management: chronic kidney disease, renal care, care coordination, kidney transplant or dialysis experience (home, in-center, hospital).

No holidays or weekends
