Job description:

Forever Young Adult Day Health Care Center is currently looking for a social work assistant

Qualifications:

Fluent in English/Mandarin or English/Spanish**ONLY
BA/BS in Social work, sociology, gerontology, or psychology ONLY*.
Experience working with the elderly.
Social service experience a plus.
Duties (include but are not limited to):

Conduct initial assessment and reassessments under the supervision of a MSW

Document flow sheet and progress notes

Case management- apply for access, IHSS, medi-cal renewals, etc.

Send F2F requests and contact with MCP for authorizations/denials

Follow up with PCP r/t pending H&P’s

Other duties as assigned by PD

Job Type: Full-time

Pay: $23.00 - $25.00 per hour

Expected hours: 30 per week
