About the Role:
We are seeking a dedicated Linkage to Care Manager to oversee programs connecting individuals to essential healthcare and supportive services in San Bernardino. The Linkage to Care Manager will supervise staff, coordinate outreach and case management, and ensure clients receive timely access to medical, behavioral health, and community resources. This role requires leadership, strong communication, and a deep understanding of public health and community-based services.

Key Responsibilities:

  • Oversee and manage linkage-to-care programs, ensuring clients are connected to medical, behavioral health, housing, and supportive services.
  • Supervise and support case managers, outreach workers, and other program staff.
  • Develop and maintain strong partnerships with local healthcare providers, community organizations, and social service agencies.
  • Monitor program outcomes, prepare reports, and ensure compliance with grant and funding requirements.
  • Provide training and technical assistance to staff on best practices in client engagement and service coordination.
  • Participate in community events and coalitions to strengthen referral networks.
  • Address barriers to care by advocating for clients and collaborating with service providers.

Qualifications:

  • Bachelor’s degree in Social Work, Public Health, Human Services, or related field (Master’s preferred).
  • 3–5 years of experience in case management, care coordination, or public health programs.
  • Prior supervisory or program management experience strongly preferred.
  • Knowledge of HIV/AIDS, behavioral health, housing instability, and other health disparities in vulnerable populations.
  • Strong leadership, organizational, and problem-solving skills.
  • Bilingual (English/Spanish) preferred.
  • Valid driver’s license and reliable transportation required.
