Job Overview
The Case Coordinator plays a vital role in managing and overseeing the care and support services for individuals with developmental disabilities. This position involves working closely with clients, their families, and various stakeholders to ensure that each individual receives personalized care that meets their unique needs. The Case Coordinator will be responsible for program development, relationship management, and the overall coordination of services to promote the well-being of clients.
Duties
- Develop and implement individualized service plans tailored to the needs of clients with developmental disabilities.
- Coordinate and manage programs that enhance client outcomes through effective service delivery.
- Foster strong relationships with clients, families, and service providers to ensure comprehensive support.
- Supervise staff members and provide guidance to ensure high-quality care is delivered consistently.
- Maintain accurate documentation of client progress and program effectiveness.
- Collaborate with interdisciplinary teams to address behavioral health needs and promote holistic care.
- Manage budgets related to program services, ensuring efficient use of resources.
- Conduct regular assessments to evaluate client needs and adjust programs accordingly.
Skills
- Strong management skills with experience in supervising teams effectively.
- Proficient in program development and implementation strategies.
- Experience working with individuals who have developmental disabilities is highly desirable.
- Excellent relationship management skills to build trust and rapport with clients and families.
- Administrative experience in case management or related fields is essential.
- Knowledge of program management principles to ensure successful service delivery.
- Background in social work or behavioral health is a plus, enhancing understanding of client needs.

Job Type: Full-time
Pay: $22.00 - $30.00 per hour
