- Assist individuals with disabilities, together with their families/advocates through all of the steps of the front door process meeting targeted timeframes.
- Work in partnership with front door team members under the supervision of the front door lead.
- Provide individuals/families with information/education on OPWDD eligibility process, service options, referral, and advocacy.
- Provide linkage information/assistance for care coordination organization (CCO).
- Develop a working relationship with CCO staff.
- Ensure Adherence to NY Care Coordination Organization Policy
- Host/Attend Meetings. Deliver Front Door Information Sessions
- Provide Technical Assistance to Care Coordination Organizations and Care Managers
- Maintain and engage in necessary follow-up for resolution as it pertains to Customer Services Log
- Proficiency with Information and Technology systems such Tracking and Billing System to complete overrides, Choices, PowerApps, Capacity Management Application, Sharepoint and Teams and Microsoft products
- Manage Care Coordination Organization disenrollment through necessary monitoring and review
- Foster effective collaboration and communication.
- Understand, review and maintenance of SPS/RFO Policy and Procedure Manuals
- Assist Front Door Department with Projects and Assignments
- Adherence to time and attendance rules
- Implement organization strategies in day-to-day work
- Occasional overnight travel to support attendance at trainings, meetings, conferences. At times these could occur in other areas of the state and may require overnight travel.

- Please note that starting salary for this title reflects the downstate adjustment. Posted Hiring Salary includes the sum of the statutory hiring rate for PS&T Grade 20 ($78,193) and Downstate Adjustment ($4,000). Downstate Adjustment is also added to the job rate ($98,121).
- If you are currently a New York State employee serving in a permanent competitive qualifying title and eligible for a transfer via Section 70.1 of the Civil Service Law, you may be appointed to this title as a permanent competitive employee.
- The assignment of PS&T employees will be made in accordance with any agreed upon Memorandum of Understanding between PEF and the local DDSO regarding the posting of job vacancies.
- Preference will be given to OPWDD employees impacted by closures. If you are being impacted by closure, please indicate this on your resume.
- All OPWDD employees must be eligible and maintain eligibility for full and unconditional participation in the Medicare and Medicaid programs. Continued employment will depend on maintaining eligibility.
