The role youll contribute:The RN Patient Care Coordinator is responsible for coordinating the care of patients within their specialty. Responsible for ensuring the full continuum of patient care is being met.The value youll bring to the team:
  • New patient consults and patient work up coordination
  • Scheduling with continuation of care including autologous donor collection, stem cell lab, One Blood, adult Bone Marrow Transplant team, and others as needed.
  • Surgery coordination and liaison with patient, physicians, and other physicians in the community, physician assistants, and anesthesiologists as required.
  • Initiates, facilitates and monitors the development of patient care pathways that identifies critical factors for achieving desired outcomes for discharge, post-hospitalization recovery, and health maintenance and promotion for a specific patient group.
  • Documents patients daily progress in the medical record, communicating unexpected results/symptoms to Physician
  • Performs nurse visits, and wound care to augment care given by the physician in the office or in the OR.
The expertise and experiences youll need to succeed:Minimum qualifications:
  • Associate degree in Nursing (ASN)
  • 1 year of RN
  • Basic Life Support
  • Florida State RN License
Preferred qualifications:
  • Care Coordination Experience
  • Relevant specialty experience
  • Bachelors Degree in Nursing (BSN)
