Job Description:Summary:The Social Service Coordinator provides medically related social services to attain or maintain the highest practicable physical, mental, and psycho-social well-being of each resident and provides direct services as needed.Essential Duties and Responsibilities:1. Completes and distributes all necessary paperwork for resident admissions and resident discharges2. Conducts resident assessments3. Initiates and participates in resident care plans and other family meetings4. Collaborates with other team members to address resident and family concerns5. Documents in residents’ charts regarding pertinent information to conform with EverTrue policy and state and federal regulations6. Identifies residents in need of services either through direct referral from families, other residents or staff, community incident reports, trigger reports and/or assessments and provides follow-up as needed7. Communicates effectively with home health agencies, other nursing homes, hospitals, etc, regarding residents8. May be assigned to conduct closings in AL/ALMC/Personal Care and Care Center9. Assists in supporting other departments as needed to benefit residents and familiesQualifications, Knowledge, Skills, & Abilities:A Bachelor of Social Work, Gerontology, or related degree preferredPrior experience working in long term care preferredAbility to communicate verbally with a high degree of sensitivity. Strong written communication skills and documentation skills.General understanding and knowledge of geriatricsMust be able to adjust to daily interruptions and re-arrange schedule due to resident, family, and community needs.Ability to function independently and in a team environment.Physical Requirements and Working Conditions:The physical activities, demands and working conditions described here are representative of those an employee encounters while performing the essential functions of this job.
