Requirements
As an Intake Coordinator/CNA Supervisor you will manage the entire patient intake and referral process for hospice services. This role serves as the first point of contact for new patients and families and oversees the scheduling, training, and performance of the hospice CNA team. The coordinator ensures a smooth transition into hospice care for patients while providing clinical and administrative support to the CNA staff.

Responsible for managing the patient intake process.
- Collect and verify patient information, including insurance details
- Schedule appointments and manage the appointment calendar
- Coordinate referrals to specialty providers as needed
- Answer phone calls and respond to patient inquiries
- Maintain accurate and up-to-date patient records
- Assist with medical billing and coding tasks
- Ensure compliance with HIPAA regulations and patient confidentiality
- Provide leadership and mentor CNA staff.
- Oversee the creation and management of CNA schedules to ensure adequate coverage.
- Conduct staff development, including orienting new CNAs and providing ongoing training and skills reviews.
- Monitor and ensure staff adherence to agency policies and procedures, including safety practices and documentation.
- Perform direct patient care as needed, assisting with personal hygiene, meal preparation, and ambulation.
- Monitor and provide accurate documentation of services and patient conditions.
- Support and provide emotional support to patients and families.
Requirements
- Current Certified Nurse Aide (CNA) certification
- Current Basic Life Support/CPR certification
- Preferred hospice or home care experience
- Current and valid driver's license, reliable transportation, and current liability auto insurance.
- Must be able to meet all physical demands of the job which includes, but is not limited to seeing, speaking, touching, walking, standing, bending, reaching overhead, crouching/kneeling or crawling, as well as the ability to regularly lift/move up to 50lbs independently and up to 200lbs with assistance.
- Self-motivated to accomplish identified goals with a strong sense of accountability for results.
- Previous experience in a medical office or similar administrative support role
- Proficiency in medical scheduling software (Axxess)
- Strong organizational and multitasking skills
- Excellent communication and customer service skills
- Knowledge of medical terminology and procedures
- Ability to work effectively in a fast-paced environment
- Attention to detail and accuracy in data entry
