Job Description

Minimum Qualifications

Department Preferences

  • Leads a team of care coordinators focused and connecting candidates/members to needed primary care, outpatient and social services, demonstrating reduction of unnecessary emergency department (ED)/emergency room (ER) and inpatient (IP) utilization and improved health outcomes of the population.
  • Leads regularly scheduled individual supervision and team meeting sessions with the assigned team with supporting documentation. Verify that supervisory activities are connected to system priorities, strategic pillars and performance evaluation metrics and are inclusive of staff participation. Facilitate team huddles and co-lead team meetings.
  • Utilizes data reports to evaluate performance of staff around assigned targets and quality of services. Ensures proper documentation of performance issues, as well as performance improvement plans. Provides mentoring and coaching to support staff development.
  • Monitors alerts (e.g., hospital admission/discharge, incarcerations, hospital ED visit and frequent utilizer reports) to ensure that all follow up services adhere to all governing policies and procedures. Provides support/guidance for patient crises matter and support effective completion of incident reports and closed loop communication with staff to provide coaching and support in these areas.
  • Utilizes analytics and reports to support population management and team awareness of patient needs (e.g., critical alerts monitoring, hospital admissions/discharges, incarceration alerts, etc.) through proactive and timely reports communication and follow up strategies with the team. Ensures that follow up services adhere to all governing policy and procedures (P&P) and identified best practices.
  • Leads and participates in regular case conferences with staff to provide support and guidance to team. Ensures that members are connected to appropriate level of care, prepared for program graduation, or escalated to a higher level of care accordingly.
  • Reviews and approves/co-signs documentation (including notes, assessments and care plans); coaches staff as needed to improve quality and timeliness of these documents.
  • Maintains positive partnerships with providers and clinics internal to (and external to as necessary) NYC Health + Hospitals; ensure ability for providers to extend feedback on members’ progress and coordinate member care (e.g., case conferences, grievances, etc.). Supports the design of shared programmatic goals and development/application of interventions and projects to achieve those goals.

Knowledgeable In

Health Home and/or other Care Coordination Services.

  • Proficiency/experience working within electronic health record (EHR) systems
  • Utilizing excel data reports to identify trends

Equipment/Machines Operated

Willingness and ability to regularly travel between assigned work sites.

Computer Programs/Software Operated

Proficiency/Experience in the following:

  • Microsoft Office Suite (Outlook, Word, Excel, PowerPoint)
  • Electronic Health Record (EHR) System

Years Of Experience

Minimum of 1-3 years’ experience in successfully supervising line and supervisory personnel

  • Minimum of 2-4 years’ experience in care coordination
