Job Overview
We are seeking a compassionate and dedicated Medical Social Worker to join our healthcare team. The ideal candidate will provide essential support to patients and their families, assisting them in navigating the complexities of healthcare systems and addressing psychosocial needs. This role involves working closely with individuals experiencing a range of challenges, including mental health issues, chronic illnesses, and end-of-life care.
Duties
- Conduct comprehensive patient assessments to identify social, emotional, and environmental factors affecting health and well-being.
- Provide individual and group counseling to patients and families, utilizing techniques such as motivational interviewing and cognitive behavioral therapy.
- Develop and implement treatment plans in collaboration with healthcare providers, ensuring continuity of care through effective case management.
- Facilitate crisis intervention services for patients experiencing acute distress or emergencies.
- Assist patients with accessing community resources, including hospice care, addiction counseling, and outpatient services.
- Maintain accurate medical documentation in compliance with HIPAA regulations, ensuring confidentiality of patient information.
- Participate in clinical research initiatives to enhance service delivery and improve patient outcomes.
- Collaborate with interdisciplinary teams to provide holistic care that addresses both physical and mental health needs.
- Support families dealing with dementia care, Alzheimer's care, PTSD care, and other specialized needs through education and counseling.
Experience
Candidates should possess a strong background in social work with experience in various settings such as hospitals, nursing homes, outpatient clinics, or assisted living facilities. Preferred qualifications include:
- Experience with child & family counseling and working with children or individuals with developmental disabilities.
- Familiarity with medical terminology, ICD coding (ICD-9 & ICD-10), CPT coding, EMR systems, and medical records management.
- Knowledge of behavioral health practices including crisis management, behavioral therapy, group therapy, and psychotherapy.
- Proven ability to conduct diagnostic evaluations and provide comprehensive intake assessments.
- Experience in geriatrics or pediatrics is a plus.
