About The Role

We are hiring Intake & Triage Coordinators to support a healthcare-focused program. This position is ideal for detail-oriented professionals with experience in data entry, medical terminology, and CRM systems. You will serve as the first point of contact for incoming patient referrals, ensuring accurate and timely processing.

Key Responsibilities

• Review and validate incoming referral forms for completeness
• Follow up with stakeholders to collect missing information
• Accurately enter program-specific data into CRM systems
• Create or update patient profiles and link referrals accordingly
• Assess requests and assign services based on program guidelines
• Collaborate with internal teams to resolve issues efficiently
• Maintain confidentiality of patient and proprietary information

Pay And Benefits

The pay range for this position is $16.00 - $16.00/hr.
