What We Offer

  • The initial compensation for this position ranges from $68,640 to $84,418.54 annually.
  • Salary is dependent on commensurate experience above the minimum qualifications for the role and internal equity considerations.
  • The salary may also vary if you reside in a different location than the location posted.
  • 8% License Differential for LMFT, LCSW, LPCC, PsyD.
  • 7.5% Bilingual Stipend contingent upon Bilingual assessment completion.

Who We Serve

Pacific Clinics serves children, transitional age youth, families, adults, and older adults. We offer a full range of mental and behavioral health services, foster care and social services, housing, continuing adult education and early childhood education programs to Medi-Cal eligible individuals and families throughout Alameda, Contra Costa, Fresno, Kings, Los Angeles, Madera, Orange, Placer, Riverside, Sacramento, San Bernardino, San Francisco, Santa Clara, Solano, Stanislaus, Stockton, Tulare and Ventura Counties.

About our Program:

Pacific Clinics Foster Care and Adoptions program serves youth birth through 21-years. These youth are dependents of the county and are placed with our approved resource families while they await reunification with their birth family, guardianship, or adoption. We support our resource families and foster youth to ensure placement stability and strive to bring about positive outcomes

JOB SUMMARY:

Schedule: Monday through Friday, from 8:30am - 5:00 p.m.

Works within the vision, mission and philosophy of the agency. The Social Worker I provides case management support and services typically within the Foster Care and Adoptions services to ensure the emotional and physical safety of youth and families.

RESPONSIBILITIES AND DUTIES

1. Conducts initial and ongoing assessment of youth and family strengths and needs.

2. Develops, reviews, updates and ensures implementation of strength-based service plans for each youth including safety plans, and legal issues.

3. Maintains ongoing communication and provides pertinent information to and coordinates with authorized representatives and other team members, participating in meetings, such as youth/family teams.

4. Develops permanency connections by building and supporting biological family and connections with other natural support persons; empowers resource families to make permanent commitments to children, and facilitates visits with birth parents and other relatives.

5. Supports resource parents to facilitate a successful placement and ensures compliance with agency and regulatory requirements.

6. Meets with resource parents and youth in the home and community settings, developing a helping relationship and ensuring needed supports and services are provided.

7. Facilitates support groups, provides training and participates in events for resource families, and youth.

8. Rotates service as on-call social worker for after hour emergencies.

9. Attends weekly supervision, weekly staff meetings, in-service training and treatment team meetings.

10. Maintains confidential files, including electronic information.

11. Completes all documentation and file compliance in accordance with agency and regulatory requirements.

12. Meets performance goals; e.g., productivity and quality.

13. Participates in departmental and agency work groups.

14. Performs other related responsibilities as assigned to support specific department/business needs

QUALIFICATIONS

MINIMUM EDUCATION AND/OR EXPERIENCE

  • Master’s degree (M.A.) in Social Work or Social Welfare; Psychology; Marriage, Child and Family Counseling, or Counseling Psychology .
  • Equivalent experience that permits an exception by Community Care Licensing.

Must possess a valid CA driver's license and insurance with two years’ experience and maintain an insurable driving record under the agency’s liability policy.

HIGHLY DESIRABLE SKILLS

  • Speak, read, and/or write another language.
